What exactly is ICSI?
ICSI, or Intracytoplasmic sperm injection, is a procedure involving the injection of a single sperm into the human egg directly. The technique was initially developed for helping achieve fertilization in cases of extreme male factor infertility or failed fertilization in prior attempts at IVF. With the help of this procedure, several barriers can be overcome to allow fertilization. It provides couples with little chance of getting pregnant hope of obtaining fertilized embryo.
What does the procedure of ICSI involve?
The procedure of ICSI requires the woman to take fertility medication to stimulate her ovaries to develop multiple mature eggs. Then, these eggs are aspirated with the help of vaginal ultrasound through the vagina and incubated in the embryology laboratory under precise condition. Preparation of the semen sample involves sperm washing to separate live sperm from the dead sperm and debris. A single live sperm is then picked up in a glass needle and directly injected into the egg.
Who should consider ICSI?
The treatment of ICSI is usually considered necessary when the male factor infertility is caused by an abnormal semen analysis. Even if it is not necessary, a lot of couples still opt for IVF with the use of ICSI. Other than male factor infertility, other reasons why patients choose to undergo ICSI include

We recommend you to seriously consider ICSI if the sperm test results show any abnormality. ICSI is also recommended in cases of male partners who have had their vasectomy reversed. No matter how the sperm quality is, there may be antibodies present in the sperm that can possibly affect fertilization.

If there isn’t any evidence of male factor infertility, it may get difficult to decide on proceeding with ICSI. ICSI can possibly be chosen to maximize the chances of fertilization. However, if the sperm parameters are normal, even standard insemination with IVF provides maximum fertilization chances.
What is split ICSI?
Split ICSI is helpful for couples who wish to know about their fertilization capability. With this option, we perform ICSI on most of the mature eggs and incubate the rest with sperm. However, there is a requirement of a minimum number of mature eggs being available for split ICSI to be performed. On the day of egg retrieval, we can proceed with split ICSI only if a minimum of 8 mature eggs can be identified.

What is the success rate of ICSI?

What risks are associated with ICSI?
ICSI does come with its own set of risks. Firstly, the insertion of the needle can damage a few eggs during the ICSI procedure, generally less than 5%. There is also a 0.8% risk of the baby being born with a chromosomal abnormality. This is roughly 4 times the risk with normal conception. Currently, the reason for this increased risk is unknown. Sex chromosome abnormalities are also associated with the following issues:

Higher risk of miscarriage
Higher risk of learning or behavior disabilities
Heart problems for infants that may need surgery
Higher risk of infertility during their adulthood of your children
With higher maternal age, there is a higher risk of chromosomal abnormality such as Down’s syndrome. ICSI doesn’t increase this risk. Children born after ICSI may have developmental delays, although there is no evidence to conclusively prove that.
How many of my eggs will be injected?
If you choose to undergo ICSI treatment, we will try to get as many eggs injected as possible. You need to understand that injection with sperm can only be done on mature eggs. Our IVF experts can determine if an egg has matured or not. The chances of fertilization are very low if immature eggs are incubated with sperm. Roughly, we can inject 75% to 80% of the recovered eggs.
Is there any difference in the quality of the embryo and the rate of pregnancy between regular embryos and ICSI?
When compared to the regular embryos, there is no apparent difference in the embryo quality achieved through ICSI. In the same way, there is no difference in the rate of pregnancy as well. A lot of infertility specialists do believe that embryo yield may increase from the number of eggs recovered, although there is no definite proof of it.
