Hematopoietic Cell Transplants:

1. Autologous Bone Marrow Transplants:

An Autologous bone marrow or stem cell transplant involves harvesting healthy stem cells from the patient’s own body to restore the bone marrow’s ability to produce blood cells. This type of BMT is used in the treatment of certain high-risk cancers and autoimmune disorders. It carries a lower risk of graft-versus-host disease, as the transplanted cells originate from the patient itself.

2. Allogeneic Bone Marrow Transplants

Allogeneic bone marrow or stem cell transplants involve the use of stem cells from a healthy donor. This type of transplants is required for treating many genetic blood disorders, high-risk blood cancers and other genetic disorders as well. Allogeneic transplant can be from matched related donor or matched unrelated donor and Haploidentical type, based on donor availability. This type of transplants may possess graft-versus-tumour effect in case of cancers.

Haploidentical Transplant is a type of allogeneic stem cell transplant that uses partially matched family donor (a close family member like a parent, child, or sibling), thus expands donor options for patients. This option is ideal when a fully matched donor is not available.

Cord Blood Stem Cell Transplantation:

A Cord blood stem cell transplantation using stem cells from umbilical cord blood is an option for patients without matched donors. These stem cells, collected immediately after birth, possess a remarkable capacity to differentiate into mature, functional blood cells more rapidly and efficiently than stem cells obtained from the bone marrow of a child or adult.

Why is a bone marrow transplant needed?

Bone marrow transplant is needed to treat conditions like leukemia, lymphoma, severe anemia, immune deficiencies, or certain genetic disorders.

What is the typical duration of the bone marrow transplant procedure?

The overall duration of treatment varies depending on the underlying condition and type of transplant. Pre-Transplant preparation may take 2 weeks to 2 months. The transplant procedure itself requires hospitalization for a minimum of 4 weeks. Following the transplant, intensive follow-up care is typically necessary for 3 to 6 months, with total follow-up extending over a period of 1 to 2 years to monitor recovery and long-term outcomes.

Is bone marrow transplant safe for children?

Bone marrow or Hematopoietic stem cell transplant is curative for children suffering with high risk cancers and is the only available cure for genetic disorders like Thalassemia, Sickle cell anemia and other transfusion dependent anaemias, Bone marrow failure syndromes, Metabolic disorders like Gauchers disease, Mucopolysaccahridosis, Immunodeficiency disorders etc. Specialized conditioning regimens and treatment protocols are followed for BMT in children to ensure safety and best outcomes.

What is the success rate of bone marrow transplant in children?

Success rate for BMT depends on certain factors like availability of full match donor, underlying disease and the general condition of the child undergoing transplant. BMT success rate is better in children compared to adults as most children do not have many co-morbidities and can vary from 75% to more than 95%.

What is the youngest age for a bone marrow transplant?

For certain genetic disorders like severe immunodeficiency and high-risk blood cancers, BMT is an emergency and needs to be done as soon as possible even during infancy. For most other conditions, usually BMT is done after 1-year age.

Is bone marrow transplant painful?

The bone marrow transplant procedure per se is not painful for the child.

What are the potential side effects of bone marrow transplants?

The potential side effects include: Related to chemotherapy –conditioning, Loss of hair/ skin pigmentation, Mouth ulcers/Bleeding, Infections-viral/bacterial/fungal, Engraftment syndrome, Veno-occlusion, Immune mediated Problems, Graft Versus Host disease.
